Key Responsibilities- Deliver personalized care and support to patients in their own homes, focusing on their physical, emotional, and social needs.
- Work collaboratively with our team of Registered Nurses and other Healthcare Assistants to ensure seamless care delivery.
- Monitor patients' symptoms, administer medications, and maintain accurate records of their care.
- Provide emotional support and companionship to patients and their families, promoting a sense of comfort and well-being.
- Participate in ongoing training and professional development to enhance your skills and knowledge in end-of-life care.
- Relevant experience in healthcare, preferably in end-of-life care or a related field.
- Ability to work independently and as part of a team, with a strong focus on patient-centered care.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and colleagues.
- Valid driving license and access to a vehicle for business use.
- Ability to register with the Scottish Social Services Council (SSSC) and complete an SVQ Level 3 qualification or equivalent.
