As a Care and Support Professional, you will play a vital role in delivering high-quality care and support to our residents, promoting their independence, dignity, and well-being. You will work collaboratively with our multidisciplinary team to develop and implement personalized care plans, ensuring the unique needs and preferences of each resident are met.
Your Key Responsibilities
- Provide person-centered care and support to residents, promoting their independence and dignity.
- Develop and maintain effective relationships with residents, their families, and other healthcare professionals.
- Contribute to the development and implementation of care plans, ensuring they are tailored to the individual needs of each resident.
- Participate in multidisciplinary team meetings to discuss resident care and support, and contribute to the development of new initiatives and programs.
- Maintain accurate and up-to-date records of resident care and support, ensuring compliance with regulatory requirements.
