Job Description

The Nurse Practitioner - Rehabilitation will be responsible for assessing, planning, implementing, and evaluating patient care within our community hospital setting. You will work collaboratively with the multidisciplinary team to provide evidence-based nursing care and promote independence among patients.

You will have the opportunity to develop your skills and knowledge in areas such as palliative care, tissue viability, diabetes, and dementia. Our trust offers a comprehensive induction program, which includes training on our clinical systems and processes.

The estimated salary for this role is £40,000 - £55,000 per annum, depending on experience. We offer a range of benefits, including 27 days' annual leave, flexible working options, and access to ongoing professional development opportunities.
