Job Title: Community Care Assistant: Cluster Responder

Job Summary:

We are seeking two creative and enthusiastic individuals to join our team as Community Care Assistants: Cluster Responders. As a key member of our East Neighbourhood Centre Office, you will work closely with our partners in health and be integrated within the wider Health and Social Care Partnership.

Key Responsibilities:

  • Contribute to and support professional staff within a multi-disciplinary team in managing their caseloads by carrying out a range of designated tasks.
  • Ensure departmental objectives and service user's needs are met.
  • Work effectively with staff to support risk assessment and decision making.
  • Build positive and productive relationships with colleagues and service users.

Requirements:

  • Previous experience in a range of social care and/or health settings.
  • Good organisational and practical skills.
  • Ability to build positive and productive relationships.
  • Good communication and listening skills.
  • Sound knowledge of basic IT systems.
