Care Coordinator for Young Adults in Transition

We are seeking an experienced and compassionate Social Worker to join our team as a 14+ Leaving Care Social Worker in Northumberland. This role offers the perfect blend of challenge and reward, as you support young adults transitioning from care into independence.
About the RoleThis is a dynamic and fast-paced position that requires strong communication skills, empathy, and a passion for making a difference in the lives of young people. As a 14+ Leaving Care Social Worker, you will work closely with education, housing, and health services to provide holistic support and guidance to your clients.
Responsibilities- Manage a caseload of young people aged 14+, conducting assessments to identify their needs and aspirations.
- Develop and implement pathway plans to support the transition to independent living, working closely with relevant stakeholders.
- Provide practical advice on life skills, education, employment, and housing options, advocating for the rights and interests of your clients.
- Maintain accurate records and ensure compliance with legal and professional standards.
To succeed in this role, you will need:
- Qualified Social Worker registration with Social Work England.
- Experience working with young people in care or leaving care.
- Strong knowledge of the legal framework and procedures related to care leavers.
- Excellent communication and interpersonal skills, with the ability to engage and build trust with young people.
- Ability to manage a complex caseload and work effectively in a fast-paced environment.
