About the Role

We are seeking a highly motivated and compassionate Support Coordinator to join our team at Home Group Limited. As a Support Coordinator, you will play a vital role in supporting our customers with complex mental health needs, learning difficulties, and/or dual diagnosis.

Key Responsibilities
  • Assist in the delivery of tasks based on customers' personal support plans, needs, and aspirations.
  • Work closely with external agencies and professionals to assess and engage support for our customers.
  • Empower customers to develop independent living skills, enabling them to successfully move on.
  • Support customers with daily living activities, such as making meals, shopping, budgeting, and maintaining their tenancy.
  • Carry out risk assessments, support planning, goal setting, and regular reviews.
About You

We are looking for someone who is passionate about helping vulnerable people make positive and lasting changes to their lives. You will impress us with your ability to work on your own initiative, positively influencing people from all different backgrounds.

You will have experience in safeguarding, risk assessments, confidentiality, support plans, and working collaboratively. If you are not familiar with technology, don't worry - we will support you in developing your skills.

Requirements
  • Previous experience in a similar environment.
  • Ability to drive and have access to a vehicle insured for business purposes.
  • Enhanced DBS check.
