Community Care Manager

We are currently investing heavily in transforming community mental health services across the Black Country, and we are seeking a highly skilled Community Care Manager to join our team in Wolverhampton.
Job Requirements:- Experience of working within mental health services and within a multidisciplinary team
- Knowledge and skills in risk assessment/management, assessment and care planning for both service users and carers
The ideal candidate will have a good working knowledge of psychotropic medication and effectiveness, as well as the ability to deliver a range of therapeutic interventions. They will also have previous experience of the Recovery Model and be able to support the team manager and clinical lead in delivering high-quality services.

Salary:£35,000 - £45,000 per annum.
