Job Title: Adult Health and Care Assessor

We are seeking a highly skilled and experienced Clinical Nurse Assessor to join our team in the Integrated Care Board (ICB). The successful candidate will be responsible for assessing individuals in the community, home, nursing homes, and hospitals to determine eligibility for NHS Continuing Care or Funded Nursing Care.

Main Responsibilities:
  • Conduct comprehensive assessments of individuals to determine their eligibility for NHS Continuing Care or Funded Nursing Care.
  • Collaborate with the multidisciplinary team to coordinate the assessment process and ensure that it is undertaken in accordance with current guidance.
  • Develop and maintain effective working relationships with health and social care professionals, patients, and carers to ensure that the assessment process is transparent and clearly understood by all parties.
  • Manage a caseload of patients eligible for Funded Nursing Care and Fully Funded Continuing Healthcare, including the assessment and reviewing of all care provision.
  • Provide education and information to clients, carers, and professionals on the National Framework for Continuing Health Care and NHS Funded Nursing Care.
  • Contribute to the effective running of the department, deputising for colleagues and the Lead Nurse as required.
Requirements:
  • Relevant nursing experience gained in a complex, multi-disciplinary environment.
  • Wide experience of cross-sector partnership working.
  • Experience of working with service users and carers, including case management.
  • Good understanding of the National Framework for NHS Continuing Health Care and its application in practice.
  • Ability to work within hostile environments and maintain confidentiality of patient records.
Working Arrangements:

The successful candidate will be required to travel across the ICB sites and to external organisations. A Disclosure and Barring Service (DBS) check will be required for this post.
