Job Summary:

We are seeking a skilled Care and Support Specialist to join our team within Community Services. The successful candidate will provide person-centred care and support to vulnerable customers who require differing levels of support to meet a range of complex social and health needs.

Key Responsibilities:

* Provide and promote person-centred care and support to customers in their own homes
* Support customers to maintain or improve their wellbeing
* Work closely with our Multi-Disciplinary Team members, including Social Workers, Therapists, and Community Nurses

Requirements:

* Enhanced DBS certificate
* Full driving licence and use of a vehicle for work purposes
* Training in medication administration is preferable but not essential
