About the Role

The innovative Rapid Response & Intermediate Care service is expanding across Buckinghamshire, and we are seeking a skilled Physiotherapist to join our Wycombe team.

Key Responsibilities
  • Develop and implement active case management programmes for patients with complex needs within the locality.
  • Collaborate with Advanced Community Practitioners to allocate workloads to members of the Rapid Response & Intermediate Care (RRIC) Team.
  • Participate in rotation to the North or South Squad rota as part of the role.
  • Work with a range of professionals to ensure care is appropriate and delivered in a timely manner.
Requirements
  • Valid and current driving licence and car to travel to locations throughout Buckinghamshire.
  • Demonstrate profession-specific clinical assessment, planning, delivery, and evaluation skills.
  • Knowledge of legal requirements for working with vulnerable adults and those who lack capacity.
  • Report writing skills.
Experience and Qualifications
  • Demonstrate experience in relevant clinical areas, such as Care of Elderly, Orthopaedics, Neurology, Respiratory, Palliative Care, or Long-term conditions.
  • Experience participating in research or audit projects.
  • Experience of day-to-day staff management, such as case allocation to Health Care Assistants.
  • Experience of working in a community/in-patient setting for a minimum of 2 years post-registration.
