About the Role

We are seeking a highly skilled and experienced Mental Health Practitioner to join our team in Primary Care. As a key member of our multidisciplinary team, you will work closely with patients, families, and carers to provide high-quality mental health care and support.

Key Responsibilities
  • Contribute to the development and delivery of primary care mental health services, working closely with GPs, social prescribers, and other community-based services.
  • Provide mental health assessments, interventions, and support to patients in primary care settings, using a strengths-based approach.
  • Work collaboratively with colleagues in specialist community mental health services to ensure seamless transitions and access to evidence-based interventions.
  • Connect patients with community and voluntary sector support, promoting a holistic approach to mental health care.
  • Facilitate mental health and strengths-based assessments in primary care, supporting the primary care team in accessing appropriate services.
  • Build and maintain strong relationships with primary care networks, community mental health teams, social care teams, and voluntary sector organizations.
  • Provide targeted learning opportunities for primary care staff to develop their knowledge and capabilities in mental health care.
  • Offer information, signposting, and support to patients and their families with mental health needs.
  • Foster collaborative and trusting working relationships with referring agents and primary care health professionals.
Requirements
  • Professional qualification in mental health (e.g., RMN, Social Worker, Therapist).
  • Current registration with a professional body and commitment to continuing professional development.
  • Significant experience in assessing risk and developing risk management strategies.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
