Advanced Clinical Practitioner Opportunity

We are seeking an Advanced Clinical Practitioner to join our team at Central London Community Healthcare NHS Trust, where you will have the opportunity to make a real difference in the lives of our patients.

About the Role:

  • Deliver high-quality, person-centered care to patients in community settings, utilizing advanced skills and knowledge to assess and address complex needs.

Responsibilities:

  • Develop and implement comprehensive care plans that meet individual patient needs.
  • Lead by example, promoting excellence in clinical practice and contributing to quality improvement initiatives.
  • Communicate effectively with patients, families, and healthcare teams to ensure coordinated care.

Requirements:

  • Current NMC registration (RGN) or HCPC registration (Paramedic).
  • Proven experience working in community or primary care settings.
  • Advanced skills and knowledge in areas such as wound management, palliative care, or diabetes management.
  • Excellent leadership and interpersonal skills.
