About the Role

We are looking for an enthusiastic and motivated District Nurse to join our team at Bridgewater Community Healthcare NHS Foundation Trust. This is an exciting opportunity to work in a dynamic and supportive environment, delivering high-quality care to patients who require support in their own homes.

The ideal candidate will possess excellent clinical skills, be able to work autonomously, and demonstrate a passion for community healthcare. With a strong focus on patient-centered care, you will work closely with our multidisciplinary team to provide comprehensive assessments, develop individualized care plans, and deliver evidence-based interventions.

Key Responsibilities
  • Conduct thorough assessments of patient nursing needs, identifying areas for improvement and developing targeted care plans
  • Work collaboratively with the District Nurse team to ensure seamless information sharing, effective care coordination, and continuous quality improvement
  • Develop and maintain strong relationships with patients, carers, and other healthcare professionals to promote person-centered care and support
  • Provide education and guidance to patients and their families on health promotion, disease prevention, and self-management strategies
Requirements
  • Degree-qualified Adult Nurse registered with the NMC
  • Post-registration experience in community healthcare or a related field
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders
  • Ability to travel independently across the Trust area
