Job Description:

We are seeking a highly motivated and empathetic Youth Transition Advisor to join our team. The successful candidate will work closely with care leavers, families, and other stakeholders to ensure a smooth transition from childhood to adulthood.

This role requires a minimum of 2 years' experience working with young people in a supportive capacity, preferably in a voluntary or statutory setting. You will be responsible for delivering high-quality services that promote best outcomes for care leavers, empowering them to succeed in their lives.

Your key responsibilities will include preparing, updating, and reviewing pathway plans, conducting human rights assessments, and coordinating the provision of services to meet the needs of care leavers. You will also be expected to participate in team meetings and activities as necessary.

A professional qualification is not required, but it is expected that you will undertake professional development in the role. NVQ 3 Health & Social Care or other relevant qualifications are desirable. In addition, you should possess excellent communication and interpersonal skills, with the ability to build strong relationships with care leavers, families, and other stakeholders.

Requirements:
