About the Role

We are seeking a highly motivated and experienced Physiotherapist to join our team as a Band 5/6 Community Rehab Physiotherapist. As a key member of our team, you will be responsible for providing high-quality physiotherapy services to patients in the community.

Key Responsibilities
  • Provide physiotherapy services to patients in the community, including assessment, treatment, and rehabilitation.
  • Work closely with other healthcare professionals to ensure seamless care and effective communication.
  • Develop and implement individualized treatment plans to meet the unique needs of each patient.
  • Conduct regular reviews and assessments to monitor patient progress and adjust treatment plans as needed.
  • Collaborate with other healthcare professionals to ensure effective discharge planning and transition of care.
Requirements
  • HCPC registration as a Physiotherapist.
  • Experience in community physiotherapy, preferably in a Band 5/6 role.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to work independently and as part of a team, with a strong focus on patient-centered care.
  • Knowledge of evidence-based practice and current research in physiotherapy.
