Job Description: Community Support Lead
Salary: £15.28 per hour
This exciting opportunity has arisen for a Community Support Lead to join our team in Bradwell. As a key member of our community service team, you will be responsible for managing a team of colleagues and helping people gain more independence in their daily lives.
About the Role:
The successful candidate will develop and implement person-centred support plans, conduct care needs assessments, and complete supervisions/appraisals. You will also take part in the on-call rota, although this may be subject to change depending on the needs of the service.
Key Responsibilities:
- Develop and implement person-centred support plans
- Conduct care needs assessments
- Induct new colleagues and complete supervisions/appraisals
- Complete medication competencies and audits
- Attend multi-disciplinary meetings and complete reports
Requirements:
We are looking for an experienced individual with a Level 3 in Health & Social Care and previous experience in a care management role.
