Job Title: Community Wellbeing Practitioner
Job Summary: We are seeking an experienced Community Wellbeing Practitioner to join our team. This is a Bank position that provides cover for annual leave or sick leave, with varying hours and days.
About the Role:
This role involves delivering high-quality care and support to residents across a 20-mile radius. The successful candidate will be responsible for monitoring the delivery of care, contributing to person-centred care planning, recording observations, providing support and companionship, and travelling across the area.
The ideal candidate will have a background in care, a Level 2 NVQ in Health and Social Care, a good understanding of elderly care, and C or above in GCSE English and Maths.
Salary and Benefits:
We offer a competitive salary of £25,000 per annum, flexible hours, free learning and development opportunities, mileage reimbursement, and a bonus for achieving a Good rating during inspections.

Requirements:
- Background in care
