Main Duties of the JobThe successful candidate will work within the East Elmbridge Community Service alongside registered community nurses on twilight duty, providing care to housebound patients across Surrey Downs Health & Care localities. Key responsibilities will include delivering clinical interventions as delegated by senior team members, following personalised care plans, and providing personal, social, and emotional support to patients.
Health PromotionThe Community Health Care Assistant will assist in promoting the health and well-being of patients, supporting and informing family/carers where appropriate, and maintaining accurate and clear record keeping.
Team WorkingThe successful candidate will work in co-operation with all other professionals and agencies involved in the care of the patient, maintaining regular contact with the senior team member and communicating all relevant issues regarding patient progress.
Person SpecificationThe ideal candidate will have previous experience of caring for patients, with a willingness to work towards NVQ Level 2 and learn new skills required for this post. A clean UK driving licence is also essential.
