Service Overview
We are a leading healthcare provider seeking experienced clinicians to join our Long-Term Condition Review Service. As a clinician in this role, you will conduct face-to-face domiciliary reviews for patients unable to attend their GP practice.

About the Role
Conducting long-term condition reviews is a crucial aspect of this position. You will assess clinical conditions and provide recommendations for management, ensuring patients receive high-quality care. Your responsibilities will include conducting medication reviews, evaluating side effects, efficacy, suitability, and continued need.

Key Requirements
To excel in this role, you should have experience in delivering long-term condition reviews, preferably in primary care. Previous experience or qualifications in respiratory conditions, diabetes, and/or cardiac conditions are an advantage. You must be confident in making medication recommendations and managing long-term conditions, with the ability to perform basic clinical assessments and take relevant observations.

Compensation and Benefits
This role offers a competitive hourly rate of £28-£30 per hour, along with opportunities for professional growth and development.
