We are seeking an experienced and skilled Clinical Team Lead to join our Community Nursing Team. As a key member of our team, you will be responsible for delivering high-quality patient care and leading a team of nurses to achieve excellent patient outcomes.
About the RoleThe successful candidate will have a strong background in community nursing, with experience in leadership and management. You will be responsible for assessing, planning, delivering, and evaluating patient-centered programs of care, ensuring that you practice within your sphere of competence and knowledge.
You will work closely with other healthcare professionals to develop new and innovative clinical practices, responding to emerging knowledge and techniques. Your role will also involve developing and maintaining robust communication networks with patients, carers, team members, GPs, and other professionals and agencies.
Key Responsibilities- Deliver and lead highly specialist clinical care to your own caseload and service within your speciality area.
- Assess, plan, deliver, and evaluate patient-centered programs of care, ensuring that you practice within your sphere of competence and knowledge.
- Develop new and innovative clinical practices, responding to emerging knowledge and techniques.
- Maintain robust communication networks with patients, carers, team members, GPs, and other professionals and agencies.
We are looking for a candidate with the following essential qualifications and experience:
- Registered General Nurse - Professional knowledge acquired through degree or equivalent.
- District Nursing Practitioner qualification with Non-Medical Prescribing (V300) or Advanced Clinical Skills and non-medical prescribing V300 supplemented by leadership program.
- Completion of leadership program.
- Practice assessor/supervisor qualification.
- Registered Mentor.
