About the Role:

You will be responsible for supporting individuals to achieve their personal goals and aspirations, while promoting their independence and well-being. This will involve working closely with our team to develop and implement person-centred care plans, as well as providing support and guidance to individuals in their daily lives.

Key Responsibilities:

  • Develop and implement person-centred care plans in collaboration with individuals and our team
  • Provide support and guidance to individuals in their daily lives, promoting their independence and well-being
  • Work closely with our team to ensure seamless delivery of care and support services
  • Participate in regular team meetings and contribute to the development of our service

Requirements:

  • A caring and compassionate attitude, with a strong commitment to person-centred support
  • Excellent communication and organisational skills, with the ability to work effectively in a team
  • Good understanding of the needs and requirements of adults with physical and learning disabilities
  • Ability to work flexibly, including occasional evenings and weekends
