About the Role

We are seeking a highly skilled and motivated Assistant Practitioner to join our Carmarthenshire Community Nursing Team. As a key member of our team, you will be responsible for providing high-quality care to patients in their own homes, working closely with our experienced District Nurse Team Leaders.

Key Responsibilities
  • Support the management of a delegated caseload within the Community Nursing Team
  • Provide complex care management to patients in their own homes
  • Liaise with staff, patient/carers and other agencies/organisations as required
  • Ensure effective communication between those involved in the care package
Requirements
  • Level 4 vocational qualification in a relevant healthcare associated subject/Certificate in Healthcare Support Worker Education
  • GCSE Maths and English Grade C or above or equivalent level 2 or above essential skills qualifications
  • Evidence of continuing personal development relevant to the role in healthcare
  • Able to demonstrate understanding of the HCSW Code of Conduct, All Wales Delegation guidelines and the All Wales Band 4 Assistant Practitioner Governance Framework
Desirable Qualifications and Experience
  • Local knowledge of statutory and voluntary services
  • Teaching or assessment qualification
  • Experience in a range of clinical skills including palliative care
Language Skills

Welsh Speaker (Level 1) is desirable for this post.
