">Job Summary
">We are seeking a skilled and compassionate nurse to join our community nursing team. The successful candidate will be responsible for providing person-centred care to patients in their own homes, working closely with other healthcare professionals to ensure seamless care delivery.

- Provide high-quality care to patients in their own homes, including administering medication, wound care, and other treatments as prescribed by doctors.">
- Work collaboratively with other healthcare professionals to develop and implement comprehensive care plans that meet the individual needs of each patient.">
- Maintain accurate records of patient interactions, including progress notes, medication administration records, and any other relevant documentation.">
- Participate in ongoing education and training to enhance clinical skills and knowledge, staying up-to-date with best practices and new developments in the field of community nursing.">
- Contribute to the development and implementation of policies and procedures related to community nursing, ensuring they are evidence-based and aligned with trust standards.">

">Essential Criteria

- Nursing degree or diploma (or equivalent qualification) and registration with the NMC.">
- Driving licence and use of a vehicle for work purposes.">
- Ability to work independently and as part of a team, with excellent communication and interpersonal skills.">
- Proven ability to work flexibly, including evenings, weekends, and bank holidays.">
- Experience of working with patients in their own homes, preferably in a community nursing setting.">
- Knowledge of local policies and procedures related to community nursing, including safeguarding and confidentiality protocols.">
- Familiarity with electronic health records systems and ability to maintain accurate records.">
Salary
">£30,451 - £43,041 per annum, depending on experience.
