Responsibilities
  • Work collaboratively with the Nurse Manager to establish and manage new care packages for our clients.
  • Maintain regular shifts within your assigned client caseload, ensuring continuity of care.
  • Develop and maintain a thorough understanding of each client's requirements, delivering personalized care in accordance with their care plans.
  • Contribute to the review and update of clients' care plans in conjunction with the Nurse Manager.
  • Identify and communicate upcoming staffing gaps, facilitating recruitment and onboarding processes.
  • Provide guidance and mentorship to support workers, conducting observations and identifying training needs.
  • Ensure timely and accurate recording of care and supervisory activities.
  • Prioritize the best interests of clients at all times.
  • Monitor client satisfaction, escalating concerns and producing monthly reports.
  • Engage in mandatory paid training, including achieving a QCF Level 3 Diploma within two years of employment.
Requirements
  • Hold a Level 2 Diploma in Health and Social Care.
  • Demonstrate a commitment to attaining a Level 3 Diploma in Health and Social Care within two years of joining, which will be funded by Superior Healthcare.
  • Proven experience working with individuals who have disabilities and complex health needs, such as those requiring ventilation, gastrostomy tubes, and bowel management care.
