Care Coordinator

Responsibilities
- Lead and motivate care teams to deliver high-quality care
- Develop and implement care plans that meet the individual needs of our residents
- Work closely with our multidisciplinary team to ensure seamless care delivery
- Ensure that all care staff receive the necessary training and support to provide excellent care
- Monitor and feedback on care quality to improve our services
Requirements
- NVQ level 2 in Health & Social Care is essential for this role, and the ability to complete Level 3 is required
- At least 6 months experience of being a Senior / Team Leader in a Residential Care Home Setting is essential
- Excellent leadership and communication skills are required to lead and motivate care teams
