Job Summary

We are seeking a highly motivated and enthusiastic pharmacist to join our team in West Lothian Health and Social Care Partnership. As a Clinical Pharmacist, you will play a key role in delivering pharmaceutical care in both primary and secondary care settings.

Key Responsibilities:

  • Work with hospital at home and other teams in West Lothian to provide holistic, patient-centred care.
  • Support the provision of pharmaceutical care initiatives in primary care, including medication reviews and polypharmacy reviews.
  • Liaise with secondary care acute and St John's Hospital to ensure seamless care transitions.
  • Provide direct pharmaceutical care to older people with complex needs in primary care and community hospitals.

Requirements

To be successful in this role, you will need to be a GPhC registered pharmacist with postgraduate experience working in a patient-focused clinical environment. You should have excellent interpersonal and communication skills, as well as leadership and innovation skills consistent with NHS Lothian's values.

You will also need to be an experienced pharmacist independent prescriber and have the ability to work autonomously as part of a team.
