The Role

We are seeking an exceptional individual to fill the position of Annual Health Check Liaison Practitioner, who will work closely with healthcare professionals and community teams to improve the delivery and effectiveness of annual health checks.

The successful candidate will be responsible for developing and implementing strategies to increase the uptake of annual health checks, including working with partner agencies and local communities.

Main Responsibilities

  • To provide training and education to GP surgeries and other healthcare providers on the importance of annual health checks for people with learning disabilities.
  • To develop and implement strategies to increase the uptake of annual health checks, including working with partner agencies and local communities.
  • To contribute to the development of policies and procedures related to the provision of annual health checks.

Requirements

  • A strong understanding of the physical health needs of adults with learning disabilities and experience of working with this population.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse groups and stakeholders.
  • Able to travel within the rural area, with a valid driving license and access to a vehicle.

What We Offer

A competitive salary of £50,000-£55,000 per annum, depending on experience, plus benefits and opportunities for professional development.
