Responsibilities
  • Screen clients for health and social care needs using established assessment tools.
  • Collaborate with multidisciplinary teams to develop personalized care plans and ensure timely referrals to appropriate services.
  • Provide intensive short-term support to clients and their families, promoting independence and self-management.
Requirements
  • NVQ Level 3 Health & Social Care or equivalent experience.
  • Excellent literacy and numeracy skills, with the ability to communicate complex information effectively.
  • A basic understanding of the Recovery Model and the Mental Health Act 1983(07).
