Key Responsibilities:

  • Conduct regular care professional support visits and complete ongoing competencies to ensure our care professionals are equipped to provide the highest level of care.
  • Create, update, and audit client care plans, working closely with our care professionals to ensure seamless delivery of care.
  • Support clients and their families with enquiries, consultations, and the management of the care journey, providing a high level of customer service.
  • Maintain accurate records of client and care professional interactions on our Home Instead software and People Planner.
  • Conduct client and care professional introductions, building strong relationships and ensuring a smooth transition of care.
  • Build and maintain positive relationships with clients, their families, and other professionals involved in their care, promoting a culture of empathy and understanding.
  • Support and mentor care professionals, providing guidance and coaching to help them develop their skills and confidence.
  • Schedule day-to-day changes to support with short notice sickness, ensuring our clients receive uninterrupted care.
  • Maintain regular contact with clients and care professionals, providing updates and ensuring their needs are met.
  • Participate in on-call duties as required, demonstrating a commitment to our clients' care and well-being.

Requirements:

  • Level 3 NVQ in Health and Social Care or equivalent, or willingness to work towards.
  • Experience in the care sector delivering a wide range of personal care services, with a focus on delivering high-quality care.
  • Experience of working with MAR sheets and medication in a supervisory role, with a strong understanding of legislation and regulations.
  • Passion for delivering exceptional care to our clients, helping them to live independently and happily in their own homes.
  • Good communication skills, with the ability to build rapport quickly and establish strong relationships.
  • Confidence in using care management technology, including providing support and training to care professionals.
  • Understanding of the importance of confidentiality, working within current legislation.
  • Good working knowledge of IT systems, with experience of Microsoft Office or Google Suite and virtual communication platforms.
  • Full driving license and means of transport if required within the territory to visit clients.
  • Organised and flexible, with the ability to meet the needs of the business.

Additional Information:

Salary per annum £24,250
