Clinical Care Specialist

Job Summary: We are seeking a skilled Clinical Care Specialist to join our team at Hartford Care.
About the Role: As a Nursing Care Coordinator, you will be responsible for ensuring the highest standard of clinical care is delivered to our residents. You will work closely with the management team and other trained nurses to assess, plan, implement, and evaluate quality nursing care.
Your Key Responsibilities:
- Deliver high-quality nursing care to our residents, promoting their privacy, dignity, and independence.
- Work collaboratively with the management team and other nurses to provide a happy, caring, and safe atmosphere.
- Assess, plan, implement, and evaluate individual care plans to meet the unique needs of each resident.
- Monitor and manage residents' medication, including administering medication and recording any changes.
- Participate in regular handovers with the team to ensure seamless care delivery.
Requirements:
- Active NMC registration (PIN number) and a right to work in the UK.
- Flexible and organised with the ability to work varied shifts.
- Excellent communication and interpersonal skills.
