Location: Lincolnshire Region

Salary: £34,000-£40,000 per annum (£34 p/h days, £40 p/h nights)

We are seeking a highly skilled Home Healthcare Professional to deliver exceptional patient care in clients' own homes. As a Home Healthcare Professional, you will be responsible for providing high-quality care, collaborating with multidisciplinary teams, and maintaining accurate records.

This role involves regular travel across the Lincolnshire region. You will be required to possess excellent communication skills, be able to work autonomously, and demonstrate a strong commitment to delivering high-quality care.

Key Responsibilities:

  • Deliver high-quality patient care in clients' own homes
  • Collaborate with multidisciplinary teams to develop and implement individualised care plans
  • Maintain accurate and comprehensive records of patient interactions and care provided
  • Communicate effectively with clients, families, and healthcare professionals
  • Work autonomously and make sound decisions in complex situations

Requirements:

  • Valid Paediatric Nursing qualification and registration with the NMC
  • Experience working with complex care packages and high-acuity patients
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and make sound decisions
  • Strong commitment to delivering high-quality patient care
