Job Summary

We are seeking a skilled Community Psychiatric Nurse to join our team at Service Care Solutions. As a key member of our team, you will be responsible for providing high-quality care and support to individuals in need of mental health services.

Key Responsibilities
  • Triage referrals to the service and make recommendations for the most appropriate course of action.
  • Conduct holistic psychosocial assessments in the community to identify the needs of service users.
  • Complete gatekeeping assessments to determine whether home treatment is a viable option for service users.
  • Attend Mental Health Act assessments and formulate home treatment plans when necessary.
  • Implement risk management and care plans to ensure the effective home treatment of service users.
  • Provide support with medication management and concordance in the community.
  • Offer telephone support to service users during times of crisis.
Requirements
  • Mental health nursing qualification and registration with the NMC.
  • Experience working in a community mental health team.
  • Full UK driving license and DBS clearance.
  • Significant experience working in a community setting, including experience in an Intensive Service and Mental Health Liaison team.
What We Offer

We offer a competitive rate of pay and a £250 bonus for starting a contract with Service Care Solutions. We also offer a £250 referral bonus for successful referrals.
