Job Summary

We are seeking an experienced Care Coordinator to join our team. As a key member of our management staff, you will be responsible for line managing care workers and ensuring the delivery of exceptional care services.

Key Responsibilities
  • Line manage a team of care workers to ensure they receive the necessary support and supervision.
  • Conduct regular assessments and reviews of individual services to identify areas for improvement.
  • Develop and implement effective care plans that meet the needs of our service users.
  • Maintain accurate records of service user information and care plans.
Requirements
  • Experience in a similar role, preferably within the healthcare sector.
  • Strong leadership and management skills, with the ability to motivate and inspire a team.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with service users and their families.
  • Ability to work independently and as part of a team.
