About the Role

We are seeking a dedicated and compassionate Support Worker to join our team at Radis Community Care. As a Support Worker, you will play a vital role in promoting the independence, choice, dignity, and respect of our service users with Physical and/or Learning Disabilities (PMLD).

Our Supported Living Service in Pump Piece, Hereford, offers a safe and secure home environment where individuals can live independently with the support they need. We are looking for someone who is passionate about delivering high-quality care and support, and who is committed to making a positive difference in the lives of our service users.

Key Responsibilities:

  • Deliver person-centred care and support to individuals with PMLD
  • Support individuals to maintain their independence and autonomy
  • Develop and implement individualised care plans
  • Work collaboratively with other healthcare professionals to ensure seamless care
  • Contribute to a positive and supportive team environment
