About the Role

The County Durham & Darlington NHS Foundation Trust is seeking a skilled Dietitian to join our Community team. As a Dietitian, you will play a vital role in providing expert nutrition advice and support to patients in various settings, including hospital wards, outpatient clinics, and community settings.

Key Responsibilities:

• Deliver high-quality nutrition care to patients, working closely with other healthcare professionals to develop effective treatment plans.
• Conduct assessments, provide education, and offer support to patients with complex nutrition needs.
• Collaborate with multidisciplinary teams to ensure seamless care transitions and optimal patient outcomes.
• Participate in service improvement initiatives, contributing to the development of best practices and evidence-based guidelines.

Requirements

• State Registration in Dietetics, Member of the British Dietetic Association.
• Successful completion of dietetic placement.
• Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
• Strong problem-solving skills, with a focus on delivering high-quality patient care.
