Job Description

Lancashire County Council is seeking an experienced Care Transition Specialist to join our team. As a key member of our peripatetic team, you will provide short-term support services across various regions, promoting smooth transitions from hospital care to community-based services.

Responsibilities
  1. Triage cases before hospital discharge to assess patient needs and develop effective support plans.
  2. Conduct thorough reviews, including completing DSTs promptly post-discharge, to ensure timely discharge and minimize readmissions.
  3. Manage urgent tasks while on duty, demonstrating flexibility and adaptability in response to changing circumstances.
Requirements

To succeed in this role, you will require excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment. A strong understanding of healthcare systems and processes is essential, as well as the ability to think critically and solve problems.

Benefits

We offer a competitive salary of £35,854 - £41,844 per annum, dependent on experience, as well as access to ongoing training and development opportunities to enhance your skills and knowledge. Our comprehensive benefits package includes 27 days annual leave, plus bank holidays, and a generous pension scheme.
