Job Overview

Cwm Taf Morgannwg University Health Board is seeking a highly skilled Occupational Therapist to join our Stay Well At Home Team. This exciting opportunity involves working closely with Physiotherapists, Social Workers, and Support Workers to provide a rapid response service to individuals attending the Emergency Care Department, Clinical Decisions Unit, Acute Medical Unit, and wards.

The overall aim of this service is to prevent unnecessary hospital admissions and facilitate timely discharges using a discharge-to-assess model.

About the Role

You will work as part of a multi-disciplinary team to deliver a 7-day week service, with staff expected to work weekends on a rotational basis. The successful candidate will be required to work 3 x 12-hour shifts per week, with an additional 6-hour make-up shift every 4 weeks.

Main Responsibilities

This role requires excellent clinical reasoning, time management, observation, and communication skills. You will need to demonstrate a genuine interest in achieving wellbeing and positive health outcomes for the population we serve.

Requirements

• Degree/Diploma in Occupational Therapy
• Registered member of HCPC
• Experience of fieldwork education
• Knowledge of a broad range of medical, surgical, and orthopaedic conditions
• Knowledge and understanding of factors influencing discharge planning
• Postgraduate training in a relevant area
• Knowledge of standardized and non-standardized assessments for use with service users

Salary Information

The salary for this role is estimated to be around £35,000-£40,000 per annum, depending on experience.
