Job Overview

We are seeking a highly skilled and experienced Mental Health Nurse Practitioner to join our team as part of the Community Urgent Care Response Teams in Primary Care. As a key member of our team, you will work closely with Physical Health Urgent Care Team colleagues to provide specialist Mental Health support to patients with complex or challenging needs.

Main Duties of the Job
  • Exercise professional autonomy and critical judgement to make decisions that utilize knowledge, skill, and attitudes gained through studying and experience.
  • Provide specialist Mental Health skills advice and guidance to health professionals and colleagues, including care homes and those patients within Physical Health Virtual Wards.
  • Develop and implement education and training programs for professionals in understanding the needs and support for Older Adult Mental Health patients, including families and carers.
  • Undertake Independent Non-Medical prescribing where clinically appropriate, utilizing NICE Guidance and Trust prescribing policy.
  • Develop coordinated links throughout the Trust and wider health and social care community to meet the needs of patients and families during the referral, assessment, and signposting phase for secondary Mental Health referrals.
Person Specification
  • RMN qualification
  • Postgraduate experience working in Older Adult MH Community Services
  • Specific training/experience in working with Older Adult Mental Health
  • Effective risk assessment and management skills
  • Advanced knowledge and application of current best practice in OAMH assessment
  • Ability to build and maintain effective working relationships with team and partner agencies and other health colleagues
