About the Role

Active Care Group is seeking a highly skilled Care Coordinator to join our team in the Thame area of Oxford. As a Care Coordinator, you will play a vital role in ensuring the delivery of high-quality care services to our clients.

Key Responsibilities
  • Coordinate and schedule care packages to ensure continuity of care for clients in their own homes.
  • Provide exceptional customer service, responding to client and worker inquiries, and resolving any issues that may arise.
  • Collaborate with the care team to ensure compliance with company policies and procedures, regulatory requirements, and exacting standards of care.
  • Communicate effectively with staff, clients, and stakeholders, providing complex and sensitive information as needed.
  • Identify and implement innovative ideas to improve care delivery, demonstrating vision and supporting a creative and responsive work environment.
Requirements
  • Experience as a Care Coordinator or in a related field.
  • Previous experience using booking and phone systems.
  • Ability to handle conflict and difficult situations in a calm and supportive manner.
  • Strong work ethic and a dynamic, flexible approach to the role.
  • Excellent customer service and communication skills.
  • Ability to work independently and as part of a team.
