Integrated Care Team Manager

We are seeking an experienced Advanced Clinical Practitioner to join our team as a Senior Clinician within North West Boroughs Healthcare NHS Foundation Trust. In this challenging and rewarding role, you will have the opportunity to develop your skills and expertise, working closely with multidisciplinary teams to deliver high-quality care to patients with complex needs and long-term conditions.
The estimated salary for this position is £62,000 - £82,000 per annum, depending on experience and qualifications.
About the Role
As a Senior Clinician, you will be responsible for assessing, planning, managing, and coordinating care for patients within a defined caseload. You will work closely with medical, nursing, allied health professional, and volunteer services across primary care, secondary care, and community settings to ensure patients receive appropriate investigation, intervention, and care planning. Key responsibilities include:
* Conducting thorough assessments of patient needs and developing comprehensive care plans
* Providing expert clinical advice and guidance to colleagues and other healthcare professionals
* Prescribing medication and managing polypharmacy and other medication issues associated with chronic disease management and care home residents
* Improving patient self-management of their condition, taking into account functional and cognitive assessments
* Maintaining accurate and legible patient notes in accordance with Trust and national professional policies and guidelines
Requirements
To be successful in this role, you will need:
* A Master's degree in Advanced Clinical Practice or equivalent qualification
* Post-registration experience at senior level, with a strong track record of leadership and clinical expertise
* Evidence of working collaboratively with other disciplines and agencies, including experience of mentoring and teaching roles
* Strong communication and interpersonal skills, with the ability to balance service and educational demands
* Excellent written and verbal communication skills, with advanced clinical and diagnostic skills
