Job Summary

We are seeking a highly skilled Care and Support Coordinator to join our team in Kings Lynn. As a Care and Support Coordinator, you will have the opportunity to utilise your skills to influence, support and encourage others to maximise their potential and build a better future.

About the Role

This is a permanent, part-time position with 18.5 hours per week. The successful candidate will have experience or knowledge of accommodation legislation and possess highly effective interpersonal and communication skills both verbally and in writing.

Main Responsibilities
  • Manage a caseload of participants, producing personalised accommodation plans that cater to their unique needs and circumstances.
  • Conduct initial assessments, considering risk, situation, and need to ensure the best possible outcomes for participants.
  • Record progress and outcomes timely and accurately using our case management system to track participant success.
  • Maintain regular contact with participants to identify emerging risks and concerns, providing prompt support and guidance when needed.

  • A competitive salary of £24,650 - £29,000 per annum pro rata (dependent on experience), reflecting the demands and expectations of this role.
