About the Role

We are seeking a highly skilled and dedicated individual to join our team as a Community Connector – Mental Health. This exciting opportunity will see you working closely with GP Surgeries to support the development and mobilisation of integrated primary care mental health services.

Key Responsibilities
  • Provide early intervention and promote early triage, assessment, and formulation of need and care plans for patients with mental health presentations.
  • Coordinate and support patients to access a wide range of community services and resources, including social care, housing, family debt, and employment counselling.
  • Work collaboratively with a multidisciplinary team, including GPs, practice nurses, mental health practitioners, pharmacists, and clinical psychologists.
  • Develop and maintain relationships with community services and resources to ensure seamless access for patients.
Requirements
  • A Diploma or degree in a relevant subject, such as Counselling, Psychology, Mental Health, Social Work, or Health and Social Care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Proficiency in Microsoft Word and Excel.
  • Ability to travel around the area required for the role, with own transport preferred.
What We Offer

We offer a competitive salary of £28,000 per annum, 35 hours per week, and a range of benefits, including 26 days annual leave, pension, access to Simply Health, and more.
