Nurse Practitioner - Complex Care

Appoint Healthcare is seeking an experienced Registered Mental Health Nurse (RMN), Registered Learning Disability Nurse (RNLD) or Registered General Nurse (RGN) to join our multidisciplinary team in our purpose-built 49 bed Specialist Care Home located in Llanelli, Carmarthenshire. As a key member of our team, you will play a vital role in delivering high-quality care to adults with complex needs, promoting their physical and mental well-being, and ensuring a supportive environment.

Key Responsibilities:

  • Assess and evaluate residents' physical, psychological, and social needs to develop tailored care plans.
  • Work collaboratively with the multi-disciplinary team (MDT) to create effective care strategies.
  • Continuously assess and improve care needs, updating care plans and records to ensure accuracy and quality.
  • Provide clear explanations of how aging affects health and lifestyle, and offer guidance on maximising comfort, dignity, and independence.
  • Conduct daily assessments of service users to promote their physical and mental well-being and ensure a supportive environment.

Requirements:

  • Valid NMC registration as a Registered Mental Health Nurse (RMN), Registered Learning Disability Nurse (RNLD) or Registered General Nurse (RGN).
  • Experience in mental health care, particularly with challenging behaviours.
  • Strong communication, administrative, and computer skills.
  • A compassionate, person-centred approach to care.
  • Commitment to ongoing professional development and teamwork.
