About the Role

We are seeking an experienced Clinical Nurse Specialist to join our Integrated Services for Older Peoples and Acute Care of the Elderly (ISOP/ACE) Team. The successful candidate will be responsible for delivering effective comprehensive assessment and management of a designated cohort of complex frail and vulnerable older adults in our acute care services.

The ideal candidate will have relevant nursing experience within the specialty of older people and frailty care, as well as audit or research experience and teaching and education experience. A Non-medical prescribing qualification or commitment to undertake is also desirable.

The successful candidate will work autonomously and within a team to achieve key performance indicators, including avoiding hospital admissions and reducing length of stays. They will also be expected to work effectively with multidisciplinary teams and lead by example in delivering high-quality care.

Main Responsibilities

  • Deliver comprehensive assessment and management of complex frail and vulnerable older adults in acute care services
  • Work effectively with multidisciplinary teams to achieve key performance indicators
  • Lead by example in delivering high-quality care and promoting a positive patient experience
