About the Role

We are seeking a dedicated and compassionate Care Worker to join our team at The Hospice of St Francis. As a Care Worker, you will play a vital role in providing high-quality care to patients in their own homes, supporting them to live their lives to the fullest.

Key Responsibilities
  • Deliver personalized care to patients in their own homes, ensuring their dignity, comfort, and well-being are maintained at all times.
  • Work collaboratively with the care team to develop and implement individualized care plans that meet the unique needs of each patient.
  • Provide emotional support and practical assistance to patients and their families, promoting independence and confidence.
  • Conduct regular assessments and reviews to ensure the quality of care is maintained and updated as needed.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
  • Participate in ongoing training and professional development to enhance skills and knowledge in end-of-life care.
Requirements
  • Care Certificate or NVQ Level 2 qualification in Health and Social Care.
  • 3 years of experience in a similar role, with a proven track record of delivering high-quality care in a community setting.
  • Current driving license and use of own vehicle.
  • Knowledge of and experience in care at the end of life and after death.
  • Current knowledge of Moving and Handling, Health and Safety, Safeguarding, and Mental Capacity Legislation and Guidance.
