About the Role

We are seeking an exceptional Dementia Care Manager to join our team at Leaders in Care. This is a leadership role that requires a strong passion for dementia care and a proven track record of delivering high-quality care and support.

The successful candidate will have a deep understanding of dementia-related learning and support, as well as the ability to develop and implement Individual Care Plans, Risk Assessments, and Daily Assignment Sheets. They will also have strong communication skills and be able to work closely with families, GPs, and other professionals to ensure resident needs and preferences are met.

Key Requirements:

  • NVQ 4 or 5 / QCF 5 in Health and Social Care or willingness to work towards it
  • At least 4 years' experience in long-term care of the older person, including dementia care
  • Detailed knowledge of care plans and end-of-life care
  • Supervisory and management experience
  • Academic qualification in Management and Leadership or equivalent experience
  • Knowledge of adult abuse, safeguarding, and CQC regulations
  • Strong IT, communication, and organisational skills
