This role requires excellent communication and interpersonal skills, with the ability to work collaboratively with team colleagues and manage priorities of a caseload.
Main ResponsibilitiesThe post holder will work jointly with health, social care, and independent sector colleagues to monitor contracts with care homes and ensure the quality of patient safety and the safeguarding agenda. They will provide professional advice, support, and guidance to professionals and families concerning all aspects of CHC.
About the RoleThis is an exciting opportunity to join the CHC & Care Home Governance Team as a Band 7 post. The successful applicant will have experience across the areas of CHC, safeguarding, and care home governance. The position will be aligned to a locality Senior Nurse and will work collaboratively and flexibly with other localities where there are increased work demands.
Requirements- Master's degree level or demonstrable equivalent knowledge and experience
- Evidence of CPD - portfolio of professional development activities
- Professional knowledge supplemented by speciality training and experience in complex care
- Regulation & Inspection Social Care Act (Wales)
- Safeguarding Adults at Risk Policy and Procedures
The role will be based at Cwmbran House, Mamhilad Park Estate, Pontypool, but you will be required to travel to other parts of the Health Board's services, care homes, and local authority or other service areas.
