Care Coordinator Job Description

We are seeking a skilled Care Coordinator to join our team at TN Recruits. As a key member of our team, you will be responsible for ensuring the provision of outstanding care and services to clients and their families.

Your Key Responsibilities

  • Compliance with CQC regulations
  • Visit clients at their homes three times a week, developing comprehensive assessments and care plans that engage clients, families, and other professionals at each stage.
  • Utilize your professional expertise within an established framework, following company policies and systems.
  • Support caregivers by providing them with the information needed to deliver high-quality care to clients.

Ideal Candidate Profile

  • Proven experience as a Regional Consultant or Regional Manager in the Healthcare or Homecare sector.
  • A Level 3 qualification or higher is a plus.

What We Offer

Remote working after training (4-6 weeks), home-based in the South East region, with a UK driver's licence and access to your own vehicle required for client visits.
