We are seeking a dedicated and compassionate Support Professional to join our Transforming Care team at Home Group. As a key member of our team, you will play a vital role in supporting our customers to live a fulfilling life in their own home within the local community.
About the Role
- Supporting Our Customer
- Helping our customer, M, to overcome barriers and lead a fulfilling and meaningful life, reaching their true potential.
- Recognising their strengths and abilities, and helping them to take small steps at their pace, never losing sight of their dreams and aspirations.
- Supporting M using our Positive Behavioural Support Model, to promote independence and social inclusion through person-centred support.
- Monitoring his mental and physical health to identify any signs of deterioration and taking positive risk management to their health and wellbeing.
- Requirements
- Experience in a complex care setting, with a level 2 Diploma in Adult Health and Social Care or a willingness to work towards it.
- Worked as a Support Worker, supporting people with complex needs, or behaviours of distress.
- A patient and calm approach, able to adapt your style to M's needs, as well as the specific occasion and circumstances, even when they challenge us.
- Experience of positive risk management, ensuring the safety and wellbeing of customers and colleagues.
