Key Responsibilities- Manage patient data securely and confidentially
- Collaborate with health and social care providers to deliver integrated services
- Participate in MDT and care conference meetings
- Engage in mandatory training and attend courses as required
- Degree in a relevant field (e.g., nursing, medicine)
- Prescription authority and scope of practice
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
Estimated salary: £40,000 - £60,000 per annum
